Welcome to The Village at Pine Valley, our short-term senior rehabilitation and long-term nursing community. Dedicated to excellence, our Pine Valley campus has earned a five-star rating from CMS (Centers for Medicare and Medicaid Services). Additionally, Parkview Health has selected Pine Valley to participate in its High Performing Network.

 

Our rehabilitation community offers private, short-term rehab suites, as well as private and semi-private rooms for long-term care. Located on Fort Wayne’s northwest side, you’ll appreciate our convenient location near medical facilities including Parkview Regional Medical Center and Dupont Hospital.

Pastoral Care

Our full-time Pastoral Care team is available to serve the spiritual needs of our residents and their families. As a faith-based organization, LLV offers regular church services, Bible studies, and other opportunities for spiritual growth for those interested.

Activity Program

Lutheran Life Villages encourages all residents to join life-enriching programs. From regular chapel services and games, to music and intergenerational programming, our goal is to promote health and happiness through high quality, compassionate care and life-enhancing programs and activities. A monthly Activities Calendar is available for residents and visitors. Please see the front desk to obtain a copy.

Social Services

Our Social Services team helps meet the medically-related social and psycho-social needs of each resident. Families are also encouraged to utilize the services of our Social Workers regarding room changes, financial concerns, or just to talk to someone. They are also knowledgeable about other resources in the community and coordinate all discharges.
